We've got it working without a problem. I don't know details, but it was standard proxy server settings. We point to a proxyserver/CCMuser and it goes to the publisher.
The problem is that with 4.1(2) CCMuser pages are now https, and that has broken things. Our proxy admin is looking at a way to fix this hopefully.
