Configure IP phone service to add notes

I have CUCM version 11 and I would like to configure the phone service button to direct the user to simple text note (i.e. Site names vs site codes)

Anyone have an idea how to be configured

You would need to write a web app/page that conforms to the XML SDK and then create a phone service in CUCM pointing to that web server URL.
