During Call Phone Rang and Another Caller Entered Conversation

CM 3.3.3 SR3. We have been operating fine for over one year. Recently I've had two similar reports of this type of problem in same area, so Im putting it out for review, comments. One person said they were talking to a party, the phone went dead, and the caller was transferred to another phone in that dept automatically... the other person says they were talking, the phone rings, and the person is then on their call. I have no more detail, have not changed phone loads recently etc. The system has been flawless.

Any ideas as to what could cause this type of behavior?

Service --> Service Parameters --> CallManager (Advanced) --> Tone on Call Forward.

Set it to FALSE.
