When trying to add a new mailbox I recieve the error: "There are no more voice mail user licenses available. No users can be added". The license count for voice mail users shows 200 out of 200 used. I have only 40 mailboxes. The License Synchronizer shows 155 available out of 200 as does the Key Dump utility. The class of service count has not been exceeded. Any thoughts?

This could be related to our ability to write to the registry... there was a problem for a while in 2.x running on Win2K where the license service thought it needed to be able to write to the registry key even though it should only have been reading and if it couldn't it returned that there were no avaialble licenses.
Check out this point on setting up read/write access to our registry branch to see if it clears your problem:

Excellent! The procedure worked except that Authenticated Users was not originally in the Permissions list. I added it and continued on. The license count is now correct and I can add mail boxes. FYI we are at 2.4.6.135. Thanks for you help - Dan.
