Softkeys disappear when call placed on hold

I have a few phones that occasionally exhibit the following symptom.

An assistant answers a call. She goes to place the call on hold and no softkeys are present and thus she is unable to place the call on hold.

We recently upgraded from 3.3(5) to 4.1(3)SR2 hoping that the problem would disappear. Unfortunately, it didn't

The phones in question are 7960's with a 7914 sidecar. The phones are running P00307020300. The line in question is not on the sidecar however.

Has anyone seen similar symptoms and have any idea of what may be causing this?

After talking the the person having this issue a bit more, it appears that the issue that they are having is not as I descibe above anymore. It appears that it is exactly as described in this thread.
