Did you set the passwords for the CCMSysUser, and IPMASysUser?
Here are the instructions
Step 1 While browsing into the publisher database server, choose Start > Run and enter cmd to open a command prompt. Click OK.
Step 2 Enter the command, CCMPWDChanger.
The Cisco CallManager Password Changer window opens.
Step 3 In the Administrator Password field, enter the password of the user that was created to enable Cisco CallManager to access the directory.
Step 4 Click Next.
The Cisco CallManager Password Changer window displays. The User ID drop-down list box provides the following options: Directory Manager, CCMAdministrator, CCMSysUser, and IPMASysUser.
Step 5 You must change the passwords for the CCMAdministrator, CCMSysUser, and IPMASysUser accounts. From the User ID drop-down list box, choose either CCMAdministrator, CCMSysUser, or IPMASysUser.

How many Call managers you have and version?
The IPMA CTI route point is registered with the CCM?
If not then it is a authentication issue.
Did you changed the Pwd for IPMAuser using the
CCMpwdchanger in c:\dcdsrvr\bin in the publisher.
Is the AD integration working?
Have you added users in Global Directory and check if they are in AD ?
Are you able to search for users in Global Directory?

Yes. i have follow the instructions.
Currently only upgrade the publisher and using version 4.1.3. I have used the CCMPWDChanger to change the password.
Have seen the users updated at the AD.
How can i proceed from here?

Do you have a cluster? If so you need to upgrade the sub as well. Strange things can happen if you are running a pub at 4.1.3 and a sub at a different cm version.

Make sure you're referring to the version specific URL, as 4.1.3:
