I have a customer that receives an error everytime the listen to 3 or more consecutive voicemails through the phone. The error is "AVtrapConnectionHolderCvr.exe is either missing from the system directory,an incorrect version or corrupt." We have tried a full uninstall, including the TAC document which has you delete the registry entry. They are running VMO 3.13 TSP 3.04 and Unity 2.46.135.

AVTrapConnectionHolderCvr.exe is the process that actually makes the play back happen. Next time that happens, hit control/alt/delete and see what all processes are running.
If only one workstation is failing, you may want to find that .exe (just do a search for it) and delete it (actually name it .old) then copy the file in from a working workstation. If this is a problem across multiple systems, I would suggest opening a TAC case. It doesn't ring a bell, but maybe it is a new issue.
