6408 Gigabit Module Loopback Fail

I have a 6509 with a Sup1A-MSFC2 and a 6408 8 port Gigabit blade. I boot he switch up the Sup comes online then the 6408 comes online w/minorfailure. When I do a sho mod it says the 6408 is OK but I do a sho test 3 (6408 is in the 3rd slot" I get loopback failure on all 8 ports. Any help would be appreciated

Set the diagnostics to complete using "set test diaglevel complete" and try

*reseating the card

*different slot
