Is there anything to be concerned about with flash utilization this high?

You mean the flash device is 99.52% in capacity? The answer would be no but obviously you will not be able to put in anymore files in it. I would try to remove unwanted files from the flash. However, if you are referring to some other flash, can you post the output where you see the flash utilization?

At 99.52% utilization you would have difficulty putting anything else into flash. What would happen if the router wanted to store a crashinfo file? What if you wanted to store a new version of a file that is in flash (could be a new image file or could be a stored config file, or something else) and the new version was larger than the existing version?
But in terms of running the device, I do not see any serious impact of running with flash that full.

No problem as stated, but you should maintain your memory. If you have an emergency IOS/CatOS up/down grade, you want enough room on internal flash or bootflash to remedy the situation quickly.
