I do have ws-x6348 module, Sup2 with CATos 7.6(4) in Cat6513. but for some reasons the modules is showing Cardstatus as "faulty". but the module is operational in another Cat6513 with a Sup2/catos 7.6(4). The following is part of "show tech""

And what's mean "slot0: error = -24
Open device slot0 failed (bad device info block)" ?

issue the "show test 2" command. This command shows the results of all diagnostic tests performed on the module. The module must have failed one of the tests.

It shows that Line Card Fireware Status: Pass, but Line Card Diag Status for all ports is Fail.
The module is working on another Cat6513, so the module should be no problem.
Do you think it is related with software image, backplane or configuration?

If this card is faulty in one switch and works in another, then it could be a problem with the slot itself. First check and make sure there are no bent pins on the backplan in that slot.
Also, Try another known good module in that slot and check whether it comes up ok or faulty.

Yes, I think the issue is with the slots and go to RMA the chassis.
