the layout of your output is a bit garbled
but I see "Current Technology = Securityk9"
-> the license is preinstalled, you need not do anything

R4#sh license all

I don't know is that Last ACK received should be with date? I see deadline date with one year. It should looks like this?
Usage Reporting:
Last ACK received: <none>
Next ACK deadline: Jul 30 06:53:30 2024 UTC

yes that is correct, but...only if either
- the switch can directly contact cisco smart licensing portal on the internet
using the default cslu url
- or you have an on-prem smart licensing satellite (wich is reachable by the switch)
and you have configured reachability of this sattelite by
-- DNS resolving
-- configured the url
use a search engine for "smart licensing using policy" for your device
most info will be the same as for the 9200 series switch in Cisco Content Hub - Smart Licensing Using Policy

Is it some evaluation version?

License type: Perpetual
license is preinstalled
=> perpetual = means no expiration date ==> NO this is not an evaluation version
