Cisco internally has a CRD template that is filled out by SEs on the account team when requesting new development from the BU for their customers; typically it is submitted in conjunction with an Aha idea request. Are you referring to Cisco's template, or something more generic? The Cisco CRD is not filled out by the customers themselves.

Not entirely sure that I am following what you are asking for, so forgive me if this misses the mark...
In my experience, SPs publish/market a service offering, which describes what the service encompasses including pricing and SLAs. The marketing organizations within the SPs attempt to determine what their potential customers want through informal conversations with key customer contacts, focus groups, hiring market research consultants, talking to their own sales teams, etc. The SPs' sales teams then sell that offering emphasizing whatever advantage they believe they have over competitors in the marketplace such as coverage footprint, data rates, resilience, cost, SLAs, customer analytics, customer support, customer-facing APIs, geo-fencing, physical security of their facilities, etc. It has not been my experience that SP sales teams cold-call potential customers asking them "What service can we develop to sell to you?" as service development can take many months if not years. Rather, they sell the services they already have with those services having been developed according to their market research already mentioned. This is the part that I am missing: are you asking what are relevant questions that an SP should be asking when doing their market research to develop a new service offering, or are you asking what questions an SP sales team should be asking a potential customer in order to close a sale on an existing service?
SP customers, on the other hand, do send out questions to SPs in the form of RFPs and RFIs, asking the SPs to describe their service offerings according to compliance with the RFP/RFI requirements. BTW, RFPs/RFIs are also great market research tools for SPs!
