Hello Ashutosh

Most of the times, the Minor errors are resolved by physical reseat (jack out - jack in) of the card but the issue here could be resolved by just replacing the SFP on the card in slot 6. We are seeing the below error

3) TestTransceiverIntegrity:

      Port  1  2
      ----------
            U  F

          Error code ------------------> 1 (DIAG_FAILURE)
          Total run count -------------> 8
          Last test testing type ------> Bootup
          Last test execution time ----> Nov 09 2015 16:38:31
          First test failure time -----> Nov 09 2015 16:37:50
          Last test failure time ------> Nov 09 2015 16:38:31
          Last test pass time ---------> Nov 09 2015 16:37:50
          Total failure count ---------> 3
          Consecutive failure count ---> 2

Are you using a Cisco SFP or from a 3 party. If its a Cisco SFP on the card, then i would recommend you to please replace it. Also, please check your syslogs for any log error for the interface in slot 6 (something like unsupported transceiver).

Could you also please get the below output as well:

- show fabric channel
- show diagnostic events

Thanks
