Router checking availability host through track:

I can see following information from console:

How can I see track status through SNMP?

As far as I know there is no MIB for accessing object tracking via SNMP.
The best you can do access information about your Service Assurance Agent (SAA) / rtr job using the CISCO-RTTMON-MIB mib.
