Look like you hitting with below bug :

https://bst.cloudapps.cisco.com/bugsearch/bug/CSCtr57275

Workaround:
follow these steps before open TAC case.

1. write a new file to bootflash:test and/or harddisk:test by using "copy running bootflash:test"
2. read the file "test" to see if it works by using "more bootflash:test"
3. run command "fsck bootflash:" or harddisk and make sure all errors are fixed
4. re-run steps 1 and 2.
5. if issue persists, open TAC case. Capture the console log and add to TAC case.

Thanks balaji, for your help!

Just one thing..

Does the  "fsck bootflash:"command,  any affectation?

yes, if the box in production envirionment, do this in change window as it may have side effect to loosing the information.

if this arleady break fix, no other option, then good to go with that command to fix.

I'd upgrade to the most-current version, like 16.6.6, instead of using a re-build.

Hey Leo,

Thanks, I'll consider this.
