SG200-08p PoE ports 3&4 not working

I have been testing all possible issues with this sg200-08p 

I have 3 PoE cameras that are connected to the switch. Only ports 1&2 are up and are sending power to the cameras. I tested to see if the camera was the problem but when I plugged camera 3 into either port 1 or 2 it started up just fine. I've turned off green ethernet on all the ports as well. 

The summary page shows: 

Maximum Available Power:
0W
Threshold Power: 0W
Consumed Power: 0W

even though 2 cameras are currently running.

the log file reads this:

One last note. The port lights are green instead of amber on ports 1&2. 

Any help would be greatly appreciated.

Have you tried a firmware upgrade on the switch?  I note this model has 32W of PoE.

So you could power 4 ports of 7.4W, or if the cameras use 11W or more then only two ports (as a third port would need 33W - more than the PoE budget).

How much power do your cameras use?

Tried the firmware upgrade and defaulting the switch. The cameras use 5w each.
