2960X BOOT FAIL after reload command

I built a stack of two WS-C2960X-48TD-L and upgraded them to c2960x-universalk9-mz.152-2.E6.bin, which would get stuck during boot at some Point after loading the Image.

Then I tried c2960x-universalk9-mz.152-4.E4.bin which worked for one of the switches in the stack, but now the other one shows only "BOOT FAIL" after issuing the reload command and needs a hard reset via power cable to boot up normal. Not sure if this is just another known IOS bug or the switch is damaged.

Any ideas? Thanks in advance.

If the switch worked fine before the upgrade and it needs a hard reboot now after the upgrade, that is most likely a software issue.  That said, I have seen cases where the switch needs to be power off and than on before the new IOS can load correctly.
