I have 2 2960Xs in a stack. One switch has a pair of 1GB sfps up and working normally. I need to add another sfp to the second switch, but when I insert the sfp and do a show int is shows hardware not present, goes for both ports. Doing shut/no shut on the port(s) doesn't change a thing. I can't find much other than reloading the switch to get it to recognize the sfp. I've never had this happen before. Stack is running older code, 15.0(2)EX5.Switching

Are using a supported Cisco SFP? usually, as you mentioned, no reload is required.
