This happens when entering the command: software install file usbflash0:cat3k_caa-universalk9.16.03.03.SPA.bin new force on-reboot

First load the IOS into flash than try this command:
software install file flash:<name of the ios>

Issue appears to be that the booted path is : version3.0.7 instead of the packages.conf even though the boot variable was set to flash: is still looks at the pervious booted path.
I found a copy on flash-2: and copied it back to flash:
Everything works now.
Well except I run out of flash free space now. :-(

1. Run SW002#more packages.conf command and delete all .pkg files associated along with packages.conf file
2. Then, run command "software expand" not install in this case. Example.
SW002#software expand file flash:cat3k_caa-universalk9.16.03.06.SPA.bin [or your required bundle file]
I would suggest you to copy bundle file (.bin) from usbflash0: to switch flash: before proceeding.
