Cat 3650 Password recovery process not working

I have a Cat 3650 that was shipped to me that was on TACACS (probably) and is asking for username/pw.  I booted the switch to RMON and followed the password recovery steps, and verified the set commands (see below). It still asks for username & pw after boot flash:packages.conf is issued.

I don't care about the old config info....just need to wipe the switch.

Ideas?
