Catalyst 6509 boot looping

I have a catalyst 6509-E that boot loops every 3 minutes.

Here are the SUP cards installed:

IN SLOT 6 NOT RECOGNISED 2.1  S/N: SAL094235DY  MODEL: WS-STP720 3B
sup other 2.3 HARDWARE REVISION

POLICY FEATURE CARD 3 IN SLOT 5 SAL0942313B REV 2.1 WS-F6KPFC3B

SLOT 6 OTHER UNKNOWN - DAUGHTER POLICY 3 SAL1026SRT 2.3 WS-F6K-PFC3B

Is there any reason why this may be happening?

If the sup is not being recognized by the hardware, you may have a faulty sup. You can also upgrade the IOS to eliminate any software issues the device may have.

Minimum software requirements = 15.3(1)SY and above. Make sure your code is high enough.

Console into the supervisor card.  Post the entire boot-up sequence.  We want to see the error message(s) generated.
