Raise a TAC Case and get them to confirm if you're hitting CSCvf24186 or not.
You could try this work-around:
1. Force the appliance to boot into ROMmon;
2. Download a different IOS version, say 15.0(2)SE12. Download the BIN file;
3. Copy the BIN file into a a USB flash drive;
4. Force the switch to boot the BIN file from the USB flash drive using the command "boot usbflash0:filename.bin".
5. Once the appliance boots up 15.0(2)SE12, reboot the switch and see if the intended IOS gets loaded or not.
