Cisco 3850 iOS 12xs upgrade to cat3k_caa-universalk9ldpe.16.12.12.SPA.

Attempting to upgrade the Cisco 3850 12xs, it has been wiped and now attempting to in the last iOS cat3k_caa-universalk9ldpe.16.12.12.SPA.bin. When i attempt to install via a usb to flash: it fails with read-only error. 

Fails when attempting to install with boot to usbflash: it fails.

Fails when attempting to install via TFTP server as the ip address is SET but I can't ping to the install fails.

Can someone assist.

 USB is the best option. Just make sure you format the PENDRIVE  to FAT first and it can be no more than 4Giga

Wow thanks Flavio, I kind of thought that was the issue but wasn't 100% sure. I will try that next, thank you so much you are the man. I'll keep you posted.

Ok @flavio thanks for sharing the information on uploading the .bin file to the 3850 but here's the current problem. Whenever I upload the .bin file via usb, tftp, then reload the switch it removes the .bin file from the "flash" storage. How can I be sure that I don't have an issue with my flash storage on this switch? Thanks in advance.
