Cisco c9300 failed to boot - occurred second time.

What’s the root cause of this, if anyone can help? 

Cisco c9300 failed to boot - occurred the second time when we were doing a software upgrade. Just one switch failed out of 5 from the stack.

Contact Cisco TAC and organize an RMA because the switch is dead.
