Hi,

Is this happening to only few ports in this blade?

You may want to reset the blade and see if the issue still persist.

Hi,

When this happens, usually the blade is gone bad.  You can take the blade out of the chassis and insert it back in and see if there is a change.  If not, you may have to RMA the blade.
