Cisco IP Phone Issue

On occasion a Cisco IP phone would disconnect and re-register. It would take anywhere from a minute to a few minutes. Sometimes physical disconnecting or reconnecting is required. 

Phones: 7841 or 7861

CM - 11.5.1

Firmware on Phones - sip 12-5-1sr1-4

Logs on 2960 Stack when issue occured

050255: Sep 28 08:29:59.365 EDT: %SWITCH_QOS_TB-5-TRUST_DEVICE_LOST: cisco-phone no longer detected on port Gi1/0/31, operational port trust state is now untrusted.
050260: Sep 28 08:39:26.906 EDT: %SWITCH_QOS_TB-5-TRUST_DEVICE_DETECTED: cisco-phone detected on port Gi1/0/31, port's configured trust state is now operational.

  1. What is the exact firmware version of the switch?
  2. Post the complete output to the command "sh interface Gi1/0/31 controller".
