i used to have IOS issue with my 4506-Sup6-E Switch,i was able to recover it using a TFTP server but it still goes to ROMMON mode every time it reboots. i tried changing the cofig-register to 0x2102 but it can't take any effect either.
Can you help me how to permanently fix it? here is how it looks...

Do you have redundant supervisor cards? If yes, can you check the config register in the standby supervisor card.

Console into the switch and reboot. Post the entire boot-up process.

Ok Here are the steps i did to boot it from the TFTP as that was my only option.

The config reg 0x2102 at next boot will try to chose the ios stated in the bootvar, as apposed to 0x2101 that will pick the first ios in its sup-bootlflash no mater what is stated in the bootvar:
can you post:
sh bootvar
sh run | sec boot
or you can try and change the config-reg back to 0x2101 renamed the old bin file or make sure the bin file currently in the sup-bootflash has the correct name.

Not really sure if this model uses 0x2102, based on Cisco website try with 0x0102 as well. It can vary on some models. For example 4948 is using 0x1 as I remember.
Note For a successful ROMMON upgrade, the config-register must be set to enable reading the boot system commands (i.e. the last digit of the configuration register is set to 2; for example, 0x0102.)

In this example, we assume that the console port baud rate is set to 9600 bps and that the config-register is set to 0x0102.

Ok, I can see several issues. Firstly, the boot variable statement is incorrect. The correct boot variable statement (for a 4500E chassis based system) is "boot system flash bootflash:IOS_filename.bin".
Next, in regards to config-registry value of 0x2101. When a brand new supervisor card (for a 4500/6500) comes out of the factory the config-registry value is 0x2101 by default. This value means "ignore the boot variable statement and boot any valid IOS".
In your output, the supervisor card attempted to boot ANY IOS in the bootflash but was unable to.
Can you please post the complete output to the command "dir bootflash:"?

I did the change and this is what happens. and still gets to ROMMON mode in every reboot.
