I have a MAC address that is duplicated on 6 different vlans showing on the core switch. I am wondering why it shows up in the mac address table with this many entries when the server has only one nic and is only connected in one port. Have attached information in text provided. Can someone explain to me why this is occurring?

My assumption here is these are SVI (VLAN)interfaces on a Distribution switch as they are all on different VLAN's. This is perfectly Normal. Connect to the distribution switch and have a look at each of the SVI interfaces with the command 'show interface'

This should explain why.
