POST: CAM test failed
POST Failed: disabling stack links and shutting down SDP

driver class subsystem initialization failed

Based on that output, it looks like the switch is failing hardware tests at boot-up.  Contact Cisco TAC (1-800-553-2447) to RMA the switch.

I agree.  The switch is dead.  Contact Cisco for RMA.
