High input error rate

I have a Cisco 2960S which connects to a core 4500X switch. When I first inserted fiber cables the 2960 was having a link flap issue. That was resolved by using a different sfp module. However there is now a high input error rate. Not just in the 2960, but the port in the core switch is also having a high rate of input error. 

This is from the 2960S

Replace the fiber and/or SFP between devices, assuming that you don't have any patch panel in between, otherwise ask to check the fiber between patch panels as well.

Hi 

I do have a patch panel in between. I've already tried replacing the fiber and SFP which connects the switch to the patch panel, but it does not resolve the issue. On the 2960S I also have low Optical Receive Power, however this problem is not present on the core switch.

errors in both SW is relate to SFP and/or fiper optic you use 
what is exact model of SFP you use ?
are it cisco or other vendor ?

We are using 10GBASE-SR SFP modules. They are from cisco.

I have already tried replacing both multiple times, yet it does not stop the input errors

for this SFP you can use 30 m fiper optic cable no more

so that would include the length of the fiber backbone too right?

if the length is more we see power optic low and 0 (N/A) and this what I see from show controller

But wouldn't I see low optical receive power in the core switch too? It should be a 2 way thing right, since both the core and the 2960 are sending and receiving data.

I don't know the length of the backbone, but visually it seems well over 26m. To connect the switches to the patch panel itself a 3m cable is being used. That datasheet says with an OM3 cable up to 300m is supported, and we are using an OM3 cable. 

Result from the matrix of compatibility. This switch is supported.
