Remove the current configuration; in config mode for both switches, remove the provisioning config, on master:

no switch 2 provision ws-c2960x-48fpd-l
Power off the switches. Remove and reseat the stack modules. Power up and reboot the switches with no stack cables connected. After reboot has finished, set the priority of the switch you want master:

switch 1 priority 15
Now connect stack cables port 1 master to port 2 slave and port 1 slave to port 2 master. Slave will now reboot and the stack is formed.

Hi,

Provisioned

Ports are assigned in the stack running-configuration, but no switch with the port numbers Fa3/0/1, G3/0/1, and so on, is detected in the stack. If a switch that previously joined a stack is removed, the port numbers remain in the running-configuration, and the missing switch is shown as Provisioned.

The Provisioned state might be caused by a switch that is no longer connected to the stack or be the result of manually pre-provisioning the related ports. See the Catalyst XXXX or the Catalyst XXXX-E switch software configuration guide for more information on preprovisioning switches in a stack.

Please confirm that below points:

1. Stacking cables are not properly seated
2. Faulty stacking cable
3. Faulty stacking port

There is a bug in Software 15.2 related to stack module. Please connect the console cable on switch 4 and check the Inventory. Is the stack module is showing in inventory? If not, Unplug the stack module and connect again, Update the IOS version.
