Output failure in Distribution switch cisco WS-C4506-E

we have faced Output failure in Distribution switch cisco WS-C4506-E. after restarting the distribution switch the network back to stable. Syslog of the switch given below. Kindly help me to finding  the reason.

First picture shows the fan tray was deliberately removed.  

Second picture shows the power supply's "Output Fail" LED lamp to be ON.  This means that the power supply could be faulty or not inserted properly.

...  or the chassis really is overheating and has exceeded the set threshold and shut down?
