Guys we have Catalyst Express 500 switches which has Cisco IP phones connected to it. Users this morning complained that their PC and phone went out of order basically it lost connectivity and then got connected within minute. When I did a show log on this switch I got the following error.

I guess the switch is crashing and this is what is causing all the phones to reboot. Any suggestions to solve this please.
BTW I have corrected the time on the switch now so dont bother for the time mentioned in the logs.

Forgot to mention the IOS, we are using CE500-LANBASE-M, Version 12.2(25)FY. I checked on the Cisco website there is some ce500-lanbasek9-tar.122-25.SEG6.tar available for download. Is this issue related to a buggy IOS? If yes only then I shall consider upgrading. There is also some commercial switch patch tool available, no idea what that is. I did little bit of research on this support forum for Data TLB exception, seems like there is no solution for this as the original requestor has not commented back with the solution.

This issue was resolved by upgrading the IOS to the latest one available on Cisco download section. No more restarts, no more TLB misses.
