SFT Modules are not recognized by Cisco WS-C3850-12S switch

We installed 2 WS-C3850-12S switches and 2 WS-C3850-24T  switches recently. 

Stacked 4 of them and stack works fine. However when SFP modules (GLC-SX-MMD=) connected into 12s switches, ports get error disabled randomly. Some of the ports working fine, but others remain in shut state. We changed SFP modules, tried "errdisable detect cause gbic-invalid" command but still no change. 

Anyone had same issue or know the reason for this ? I'm sure SFP modules are genuine.

Description
Symptom:
certain vendor sfp on OIR force ports to err-disable,with below message
*Sep 10 01:29:41.353: %PM-4-ERR_DISABLE: gbic-invalid error detected on Gi1/1/3, putting Gi1/1/3 in err-disable state
*Sep 10 01:29:37.185: %PLATFORM_PM-6-MODULE_REMOVED: SFP module with interface name Gi1/1/1 removed
*Sep 10 01:29:41.353: %PLATFORM_PM-6-MODULE_ERRDISABLE: The inserted SFP module with interface name Gi1/1/3 is not supported

Conditions:
OIR of certain vendor SFPs forces the port to err-disable

so Far, we have seen issue with below vendor

GLC-T:
1.CISCO-AVAGO --puts ports to err-disable upon OIR

10G-SR/LR/LRM and 1G fibers:
1.exclight(ECL) ---puts port to err-disable upon OIR

Workaround:
shut /no shut the port
