It looks the file is corrupted. Execute dir flash: and check the size of the file and match it with the file size in the CCO.

My money would be on a corrupt IOS.
Firstly try downloading again and copy over to the switch.
If that still fails, try a different IOS version, i.e one up or one down.
I had an issue with a 3750X and a 15.* image and it would not boot for love nor money. Tried the IOS down from the one I was using and it seemed fine....weird but sometimes you get odd things happen

Time doesn't need to be synch or correct. Output shows the file is OK.
Can you boot this file: c2960s-universalk9-mz.122-58.SE2.bin
If you can, then try a different IOS. Try 15.0(2)SE4.
