I have bought 10 brand new WS-C4510R+E. SUP 8E-10GE and 8 x 4748 blades per chassis.
At first it ran without a problem, then the ports stop giving out power for the phones.
I have enough power to support them.

The Ethernet data part still works 100% if you connect a computer directly. Just the power for the phone disappear. Tried different class Cisco phones. I deleted the configuration on the port, made it a switch port again, shut/no shut. You call it, I've done it. Directly connect the phone to the switch nothing works. If you remove a active phone working 100% on the same blade and reconnect it back, it also can't start up again. At some stage the port tells you it gives power out, but there is no power coming out of the blade on the phone directly connected to the switch blade. All 3 installed switches started doing it on different blades.
Reload the blade and all is fine for about 1 months, then it starts randomly gain. Once reloaded, you can plug and unplug as you want, all is fine then.
3 switches, same problem, same solution. How can we fix this?

I have an almost identical issue. The only differences are that I'm using WS-X45-SUP7-E, and IOS 03.04.01. However, the condition of the fault is exactly the same.

You won't believe. I installed the new software, the bootvar points to the new software, but it doesn't boot from the new image. I then deleted the old image and it was ready to run the new software. When I put the old image back on the bootflash and it reloaded it went back to the old one. So I deleted it again and now I'm on the new version. Will keep you updated.

Just to update. Switches still running without any problems. Software did make a difference.
