We have a C40 with no video output on all connections.
We have tried the HDMI out the DVI out and nothing.
We have also tried to access the codec via the network and we can't get in!

Has anything with the configuration changed recently? Have you tried several different displays and cables?
Connecting to the C40 web interface, what does it say under: Configuration > System Status > Video > Output?
As a last resort, you can perform a factory reset under: Maintenance > System Recovery. If a factory reset doesn't fix it, and you still don't get any type of output after trying different displays and cables, the C40 might have failed. If that is the case, suggest you contact TAC, and might possibly need to get an RMA.

We have tried a factory reset but it did not work and we can't get access to the web interface. The warranty expired. How do we do a factory reset and with what application and settings. How do we reload the software also ?

A factory reset should just clear the configuration of the codec, but leave the software image intact. Is the C40 not getting a DHCP address when connecting it to the network, is that why you can't connect to the web interface?
You can connect to the C40 via serial using Putty or any other terminal emulator of your choice, take a look at the document, troubleshooting-ex-series-mx-series-sx-and-c-series-codecs on pg 34. Once you can then assign a static IP to the codec if one isn't assigned using DHCP and connect to the web interface.
If you believe a software image isn't loaded anymore, you check to see if an image is present by using the command: sysetemtools selectsw, it should appear similar to what's below.

You can change to a different software image, for example "image2" with command: systemtools selectsw image2. If no software image is present, you'll have to load a new image, refer the document I linked to earlier starting on pg 128. However, I strongly suggest you contact TAC if you need to do this, if you don't have a service contract, they're not that expensive to obtain one.
