phone users, press messages, they enter the password,
but then nothing happens, silence.
does not happen all the time. so it works OK most of the time.

4.0(4) is you best bet here.
On the Unity server you will need to install Exchange 2000 SP3 and post-SP3 rollup. That will patch the Exchange 2000 System Manager which includes the MAPI code that Unity uses to talk to Exchange.
Mind you that you don't install full blow Exchange on Unity, just SP3 and post-SP3 the Exchange 2000 System Manager.
And also note that there are several versions of post-SP3 so make sure to download the latest from microsoft.com.

I am pretty sure there isn't an Exchange 2000 SP4...
If they wait long enough do they get in? I am pretty sure the answer is yes.
There are lots of reasons you can get this type of delay. Microsoft had to make several changes in MAPI for us which are in the latest Exchange 2000 post-SP3 rollup / Exchange 2003. Note that the post-SP3 rollup needs to be installed on the Unity server. Unity 4.0(4) is the first version of Unity that can use the new MAPI changes.
There is also an issue with indexing of large mailboxes, which we redesigned in 4.0(4).
Network related delay between Unity and Exchange can cause this issue. Packet loss, low throughput, long delay and so on cause login delay. For this reason we dont support Unity servicing an Exchange server over any type of WAN link.
So to summaries, 4.0(4) with Exchange 2000 SP + the latest post-SP3rollup on Unity is your best bet.
