Running CM 10.5 and receiving some reports that music on hold is no longer working. In the documentation I have reviewed from Cisco it looks like the setup is OK. In any case hoping to see where to begin with this one?
I see an MOH server setup, terminations points are registered, and files exist on the server. I'm far from an expert, so I would not be surprised if I am missing something.

You should see the .wav file corresponding to all the files you uploaded from "MOH Audio file management" page, under mohprep directory.
Make sure you have the file uploaded to all the MOH servers in the cluster. MOH files don't get replicated automatically.
Once you have the files, check for User and Network hold MOH settings, can be done at 4 places:- line, device, common device configuration & service parameters
And also verify the MRG, MRGL settings.

what do you hear if you place a call on hold? was this setup working for you ever ?
have you tried restarting the IPVMSA service. make sure the “Run Flag” is set to 'YES' on MOH server config page.
is it for internal or external calls?

I've done more testing and used the sample audio files and confirmed that internal and external MoH works. Would the naming convention used in the audio file is "Live MoH Stream", would this be the same file name in the mohprep/ directory? If yes then it appears the file no longer exists on the server. I only see an XML file with that name, no WAV.

Thanks Atul very helpful information. The other server does not have a trace of the file, no XML or WAV file exists. I'll put everything to sample for now.

I'm facing the same problem on my lab (one server) and productions environment:
1. file is uploaded
2. all codecs enable
3. Network hold is configured on line, device, common device configuration and service parameters
4. MOH is selected at the MRG and MRGL

When you place the call on hold what do you hear
1) Dead Silence
2) Beep
Remember that which audio source will be played is based of the party who presses the hold button and the other party who wants to listen the MOH should have that respective audio file in its MRG group. Hence make sure that the respective audio source is there in the MRG and that particular MRG is applied to the MRGL of the party who wants to listen the MOH.
It will be worth to try restarting the IPVMS service in the lab server.

It was RTP ports that were blocked between that specific phone Vlan and call manager vlan.
