PCA HTTP 404 errors

I upgraded our failover UM system from 3.1.6 to 4.0.5. Only one problem -- the ciscopca web page returns a "the page cannot be found" HTTP 404 error. It is set to use SSL. The SA and SM links work fine. I have went through everything in the Troubleshooting Guide chapter 18 with no luck.

The Tomcat logs don't show any errors, and the Tomcat admin page opens OK.

I just opened a TAC case, but thought I would try my luck on NetPro as well.

The TAC says we need Subscriber Inboxes, which licensing says is VMI.

We never had VMI in 3.1.6 and the Active Assistant worked, which is all we want.

Can anybody shed some light on this? Do I really need to pay for something extra in 4.0.5 just to maintain the functionality I had with 3.1.6?
