Troubleshooting Calls with Crossed Lines

Working on a CUCM 7.1.3 installation w/ a VG 2821 as a H323 gateway w/ a PRI. CUCM/VG/phones all share a common subnet. For the last few days I've had at least 3 instances where someone picks up their phone either to make or answer a call and when they pickup the handset they're connected to a conference call that is already in progress by someone else in the office. The original user who reported the issue has no shared lines and neither has a softkey template that would include Barge. I pulled the call logs trying to find a correlation of the events but didn't see it.
Anything I should be looking at? For at least 1 of the calls (an inbound call that was answered) the remote party heard the initial "hello" and then received dead air. The phones in question are both 7960s running 8.1 firmware.
The environment has been mostly static for years w/ the exception of name changes on phones and the only change made recently was the edge firewall was replaced, but the VG's default route is a L3 switch which in turn routes through the edge.
