Unable to Add / Delete users

we have upgraded to 4.0(4) and E2K3 from 4.0(1) and E2K and as of 10/3 and Unity appears to work correctly overall, but we cannot seem to add or delete users from unity (the buttons are 'greyed out'.

I have run the permissions wizard that was released yesterday.

Furthermore, I have tons of errors for CiscoUnity_DSAD Event 1017 (I have a feeling it's related):

The Cisco Unity service that monitors Active Directory (AvDSAD) is skipping a synchronization cycle because it failed to connect to domain controller server.domain.com. Ensure that the server is accessible and is configured as a domain controller, and that no DNS issues exist. The service has failed to connect to this domain controller for 13 consecutive synchronization cycle(s). After 15 consecutive failed connection(s), The Cisco Unity service that monitors Active Directory (AvDSAD) will attempt to connect to a different domain controller.

Diagnostic Info:

Domain Controller: server.domain.com

Binding String: LDAP://server.domain.com:389/ROOTDSE

Error Code: ERROR_DS_UNAVAILABLE

Description: The directory service is unavailable.

If so, you will need to get hotfix 4.0(4)_ES5. You can open a TAC case to get it.

I looked at the bug, and I actually have access to the SA webpage, I simply don't have the buttons for add / delete (as well as other problems I'm sure.)

Are there parts of this bugfix that I'm not noticing that pertain to it?

The problem you are experiencing is one of the symptoms, but is not shown on the release notes.
