Unity 4.0(4) Recorded Names Disappearing

We have a new Unity 4.0(4) installation.

When users went through the tutorial their first time logging in, they all recorded their names but some, at random it seems, are periodically losing their recorded names.

If we run a report (subscriber dump) to show who has recorded their names, they all show as recorded but for some users, if you do dial by name, Unity only indicates the extension number (as opposed to playing the recorded name) when the list of matches are played. The users with missing recorded names then re-record their names and the system works as designed again but at some time in the future, some recorded names again disappear even though the user dump indicates they have a recorded name.

Is this a known issue with Unity 4.0(4) and if so, is there a fix?

This is most often caused by a rights issue. The voice names are stored in AD and replicate back into the directory (our local file system/SQL). So what happens in this case is the voice name is there for a while but when something changes that causes replication to occur with the directory and the voice name was not properly updated in AD then an empty voice name is pulled back and it's effectively deleted locally.

check to make sure your AD schema is properly extended and the rights are properly applied for the users (or more to the point the containers these users are in). You may also be seeing errors in the event log during directory synchs.
