Unity Connection Database.error-284

I am running Unity Connection 9.2.1 and I have a user that had her name changed in her user profile.  the problem is that her profile now has her old user name and her new e-mail address.  when some calls her extension it does not go to her voice mail account, it instead goes to the greeting administrator.

I went in and tried to delete her account and got a database.error-284. I have gone through and tried removing her account in CL and was able to find her account using the db query commands but I am not able to delete the account.  when the delete command is issued it comes back and says user not found.

any help is greatly appreciated.
