Unity rings busy on 2nd call to dummy after CCM 4.1.2 upgrade

Cisco Unity is coming back busy after we upgraded to CCM 4.1.2. On devices like a 12sp+ or 7960 dummy phones that are perm CFWD to VM, after the first call goes through, second call gets BUSY until the call is dropped.

I've tried changing the busy and trigger settings in CCM.

Which CallManager version did you ugprade from?

If it was an upgrade from CCM 3.x, you will need to re-configure or recreate the voice ports. This guide should help:
