Cisco VIC 1385 not listed in vic adapter in CIMC

i have installed two numbers of cisco VIC 1385 card on UCS C240M5 server but in CIMC VIC adapter section only one card is listed. i have installed the cards in slot number 4 & 5. in VIC adapter section only slot 5 is showing. In PCI adapter i could see two cards installed but for slot 4 firmware version is not showing. OS level also cards are listed but customer wants the hardware level gap as well. i am attaching the files of CIMC snap.

BIOS version :- C240M5 4.0.1C0.02627180945

Firmware Version :- 4.0(1A)
