Controller ID: 0 Controller encountered a fatal error and was reset

Hey all,

I have a Cisco C240 M3S that I will be utilizing as an MS Exchange Server with MS Server 2012 Std Edition.  I am performing stress testing currently on the server and constantly receiving the following error:

     "Controller ID: 0 Controller encountered a fatal error and was reset"

This is an LSI 9266-8I controller card that is being used.

The following is the driver\firmware versions for the controller card on Server:

    

     Firmware - 23.9.0-0023

     Driver - 6.504.4.0

I don't believe it is a driver\firmware issue as I have another machine identically configured and not experiencing any issues.  The CMIC shows everything in a health state, but this is consistently popping up as a Critical error through the MSM software. 

Any ideas, I will be getting the controller card replaced, but just curious if anyone has any additional recommendations?

Please open a TAC case, if there is not one opened already.  This condition is being investigated under
