We have UCS 240M4 with VIC 1225 in VMware vSpere 5.5 cluster and all work as expected except with VIC 1225 issue. Ports are connected to Nexus 7k in port channel with mode ON and traffic is flowing across both ports properly (we have 4 vNICs per port).
Issue we have is if we disconnect one port from Nexus either by pulling Gbic out or by shutting down port on Nexus the port on VIC goes down as expected and traffic is flowing over remaining active port but when disabled / disconnected port come back UP under CIMC it shows as port is online but traffic do not pass over that port (in the VMware it still shows as all 4 vNic are down state). Because the port is up on nexus and VIC port is also up under CIMC VIC / general / External Ethernet Interfaces switch start to send traffic across recovered port and the packets will be drooped due to the fact that VMware still see all vNic as status Down/ Negotiate as shown in picture below.
The only way to get this vmnic up is to restart the server.

Can you send a screen shot of what your vswitch teaming settings are?
Especially the 'failback' setting, which should be set to 'yes'.
Should probably confirm the basics such as overall server HUU/Firmware level, and the esxi ENIC version:#vmkload_mod -s enic

Thanks for reply, as for Failback option it does not make a difference because ESX do not see vNIC's as status UP and as it is for ESXi down it will never fail traffic to that specific vmnic. I do suspect it is a Firmware issue and my next step will be firmware upgrade and also i need to make a planed downtime to upgrade vSphere to 6.5. Will post what is result as soon as i finish.
