ok the solution to this was a patch (4) as issues was caused by a bug on the MnT Nodes. This affected the passive ID functionality and licensing, where both would function upto 4 hours after rebooting the nodes, then cease to function again.
Okay issue was resolved. The mail server was blocking the alerts from the ISE nodes. Adding the IP of ISE nodes to mail server fixed the issue. ISE configuration and understanding was correct.
