What kind of IP phone are you using? If it doesn't support CDP you will need to enalbe LLDP on your switch for the phone to discover the voice vlan.

I had this problem last week with a VSX 8000 using H.323 to a 3515. I was able to fix it by disabling Basic Mode and H.239 in the Call Preferences menu on the VSX.HTH
